Position Title

    P/T Project 360 Case Manager

Description

    Under the supervision of the Senior Program Manager of Shelter, Outreach and
    Support Services, the Project 360 Case Manager is responsible for case
    management services to homeless youth through a partnership with the King
    County Sexual Assault Resource Center (KCSARC) grant-funded project
    (Project 360). The Project 360 Case Manager will work with youth ages 13-24
    years old who have experienced sexual trauma and/or assault. Additionally, this
    position will work in collaboration with a KCSARC therapist. The KCSARC therapist
    will work with clients who are interested in accessing trauma-specific therapy. The
    Project 360 Case Manager concentrates on supporting clients to meet their goals
    of basic needs, housing, safety, employment, and education.

Responsibilities

    A. Program Operations:
        1. Maintain a consistent caseload of 8-10 youth, and meet with each client on a
        regular basis.
        2. Assess client’s needs and develop individualized, strength-based, youth-driven
        service plans in core areas of safety, recovery, housing, mental health,
        chemical dependency, education, employment, and life skills.
        3. Connect clients to community resources, including housing, health and
        mental health, chemical dependency, education, employment, life skills, etc.
        4. Engage youth through strengths-based, trauma-informed care, and harm
        reduction approaches.
        5. Partner and collaborate with KCSARC therapist to provide coordinated care
        that pertains to client needs and goals.
        6. Act as a mentor and advocate for clients.
        7. Maintain clear, professional, and timely participant contact notes, service
        plans, progress updates, statistical records, and other documentation according
        to all confidentiality laws and contract guidelines.
        8. Work with the Friends of Youth staff of the Outreach Team and Specialized
        Client Services Team to provide effective service delivery to all clients.
        9. Other duties as assigned.

Required Skills and Qualifications

    A. Education and Experience
        1. Bachelor’s degree and two years of experience working with high-risk youth.
        2. Experience working in crisis situations and with de-escalation techniques.
        3. Experience working with homeless youth and/or LGBTQ youth.

    B. Knowledge, Skills and Abilities
        1. Ability to communicate effectively in both verbal and written forms.
        2. Knowledge and understanding of issues affecting youth who are homeless
        and resources available to this population.
        3. Knowledge and understanding of issues affecting youth who identify as
        LGBTQ.
        4. Knowledge and awareness of the impact of sexual trauma and assault.
        5. Ability to maintain professional boundaries with clients while building trust
        and respect.
        6. Ability to work independently and as part of a team.
        7. Experience working with diverse populations. Acceptance of a variety of
        lifestyles, behaviors, and cultural and spiritual practices.
        8. Ability to multi-task and prioritize in order to meet the needs of clients and
        Friends of Youth.
        9. Ability to pass a DSHS Criminal Background Check.
        10. Employees and volunteers who operate their own or Friends of Youth
        vehicles on Friends of Youth business must have a safe driving record as
        defined by Friends of Youth. Employees and volunteers who operate their
        own vehicles on Friends of Youth business must carry auto liability insurance
        that complies with the requirements of Friends of Youth.
        11. An ability to engage with diverse populations in a culturally responsive
        manger.
        12. Demonstrate a commitment to the values of equity and inclusion by
        honoring and celebrating diverse characteristics and expressions of
        personal identity.

Hours

    Part Time:  20 hours / week

Salary

    $19.72 / hour
