Position Title

    Supportive Housing Case Manager


    The Case Manager provides comprehensive case management, counseling and
    career guidance services to single, pregnant or parenting youth and their children.


    A. Program Delivery
        1. Provide ongoing career guidance and educational counseling.
        2. Provide parenting education and role modeling (with parenting youth only).
        3. Identify educational resources and job training programs.
        4. Hold a series of workshops in job interviewing and preparation, career
        exploration, etc.
        5. Help locate resources for financial aid.
        6. Assist in gaining access to on-going healthcare.
        7. Assist in providing access to transportation for youth attending school, job
        interviews, appointments, etc.
        8. Coordinate delivery of resident's case management in all areas of education,
        employment, personal and health needs.
        9. Attend monthly TLP meetings.
        10. Other duties as assigned.

    B. Clinical Tasks
        1. Conduct participant intake and assessments.
        2. Assist in the development of individualized housing plans.
        3. Hold weekly individual case management sessions
        4. Keep accurate, up-to-date participant records and collect data on progress
        toward goals.
        5. Complete quarterly program reports.
        6. Provide participant follow-up and aftercare services to those leaving the
        7. Participate in agency or professional group seminars and conferences to
        increase professional knowledge.

    C. Advocacy
        1. Develop referral network of specialized service providers.
        2. Working with other case managers, local government, schools, and some
        area employers to develop jobs and mentoring opportunities for program
        3. Work with community volunteers to develop new program resources and
        career options for participants.
        4. Advocate on behalf of homeless youth and families and advocate for
        systems change.

Required Skills and Qualifications

    A. Education and Experience
        1. Master's Degree in Social Work, Counseling or Bachelor's Degree plus five
        years of experience.
        2. Experience working with abused and/or homeless youth preferred.
        3. Experience in youth education and employment programs preferred.
        4. Education or experience in cultural competency.

    B. Knowledge, Skills and Ability
        1. Ability to work as part of a team.
        2. Demonstrated skills in re-parenting.
        3. Must have own transportation available with necessary insurance.
        4. Employees and volunteers who operate their own or Friends of Youth vehicles
        on Friends of Youth business must have a safe driving record as defined by
        Friends of Youth. Employees and volunteers who operate their own vehicle(s)
        on Friends of Youth business must carry auto liability insurance that complies
        with the requirements of Friends of Youth.
        5. Knowledge of relevant COA standards and participation in a Performance
        and Quality Improvement process.
        6. Must demonstrate sensitivity and responsiveness to cultural differences
        and commitment to the value of cultural competency.


    Full Time


    $19.72 - 21.22 / hour DOE
